Femto-LASIK and SMILE both use a femtosecond laser, but they apply it differently. Femto-LASIK uses the laser to create a thin corneal flap (replacing the mechanical blade of older LASIK); an excimer laser then reshapes the cornea. SMILE uses the femtosecond laser to cut a lens-shaped piece of tissue (lenticule) inside the cornea, which is removed through a 2-4 mm keyhole — no flap, no excimer laser.

I present Femto-LASIK and SMILE as two premium, bladeless options that both outperform old-style blade LASIK. I lean toward SMILE for patients with dry eye, athletes, and those with borderline corneal thickness. I lean toward Femto-LASIK for patients with moderate-to-high astigmatism, those who want the fastest next-day vision, and anyone who values having an easy re-treatment option down the road. The good news is that both are excellent — the choice is about tailoring the procedure to the patient, not picking a 'winner.'

  • I recommend SMILE when a patient has dry eyes, thin corneas, or plays contact sports — the flapless, nerve-sparing keyhole approach is objectively safer for these profiles.
  • I recommend Femto-LASIK when a patient has significant astigmatism (−3D to −6D), wants the fastest visual recovery, or prefers a lower-cost bladeless procedure with an easy re-treatment path.
  • The mistake I see most is patients assuming one technology is 'newer and therefore better' without understanding that SMILE and Femto-LASIK are designed for different eye anatomies.
